On June 15, 2015, St. Martin Parish schooling Based health Centers reported a cut at one of its clinics, Cecilia schooltime Based Health center (CSBHS). The covered entity (CE) experienced a hack of protected health information (PHI) affecting 3,000 individuals when iv desktop computers, one laptop, a wireless router, and several printers were stolen during an office break-in on april 30, 2016. The types of PHI involved in the cut included names, addresses, dates of birth, social certificate numbers, diagnoses, and procedure codes. The CE provided cut notification to HHS, affected individuals, and the media. As a result of this incident, the CE conducted a post-incident risk analysis and directed staff to exchange and update all passwords. The ce also remotely disabled the login capability for each computer. The ce improved physical security at the CSBHS facility. in addition, the ce stated that no data is stored locally on its computers.
