Multnomah County in oregon is notifying some applicants for the or health plan whose information may have been accessed during a break-in. The break-in occurred spell the Health center was closed for twist on August 4. Files in the cabinets included applicant records with name, date of birth, address, email address, social Security number, indemnity number, information about minor children and copies of the applicants drivers license, passport, Medicare card, utility bills, permanent resident card, birth certificate, consular ID, tax statements, certificates of naturalization, and pay stubs.
